Coordinated, personalized, whole-person support

Enhanced Care Management is a free Medi-Cal benefit that adds coordinated, personalized, whole-person support for individuals and families navigating complex health or life circumstances. For many of the children, youth, and families connected to TLC, ECM complements the services they are already receiving by helping coordinate care across providers and systems.

We understand that navigating healthcare, behavioral health, and community resources can be overwhelming, and our team is here to help families access the support they need through this process.

What ECM provides

Each person enrolled in ECM is connected to a Lead Care Manager (LCM), a single point of contact who collaborates with other service providers to form a care team that coordinates medical, dental, mental health, and social services; navigates benefits and referrals; and helps to secure food, transportation, housing, and employment support.

Care managers may also help individuals to navigate school and educational resources, including support with junior college enrollment, and assist with practical needs like obtaining a driver’s license or state ID.

Care managers meet people where they are, whether at home, in the community, or at a TLC office. They center each individual and familyโ€™s own priorities in the care provided to promote overall health and wellness.

Who EMC Serves

ECM is available to Medi-Cal beneficiaries of Kaiser Permanente and Partnership HealthPlan of California.

TLC serves eligible individuals who would benefit from additional support to address mental health challenges, substance use, housing instability, homelessness, and the effects of foster care or justice system involvement. Current and former foster youth may qualify up to age 26, while other populations of focus qualify up to age 21. Support also extends to family members.

Partners and Availability

Many of the youth and families served by TLC’s diverse programs also qualify for ECM. Our team works closely with referral partners across all of our programs, including our Foster Family Agency (FFA), transition-age youth housing programs (THP), behavioral health services, and Journey Academy, to identify individuals who may benefit from ECM and connect them with this additional layer of coordinated, personalized support.
