Program Description:

Transitioning to adulthood is hard for anyone, but can be especially challenging for our most vulnerable current and former foster youth. The Transitional Housing Program (THP) and Transitional Housing Placement Plus program (THP+) provides independence with a supportive safety net. THP helps youth, ages 16-21, to rise up from foster care successfully by providing a safe housing environment  to practice the life skills necessary to live independently and work on self identified life goals. THP+ gives housing support and assistance in independent living skills to former foster youth, ages 21-25. TLC partners with landlords and homeowners throughout Sonoma County to provide housing for our youth to live. Participants receive comprehensive mobile case management from our dedicated staff with employment, health and wellness, education, finance management, accessing resources, and navigating the ins-and-outs of learning to live independently.

Position Summary:

TLC Child and Family Services is seeking a full-time Bilingual (Spanish/English) Social Worker for our Transition Age Youth Housing Program. Under the supervision of the Program Director, and reportable to the Chief Executive Officer of TLC, it is the responsibility of the Social Worker to monitor and supervise each participant’s participation in the program. The primary functions of the Social Worker are to uncover barriers to meeting independent living skills and goals identified in the participant’s youth centered and led goals assessment plan, coordinate services, link necessary resources, attend to steps needed to meet the goals assessment plan, and the ongoing daily challenges of independent living. Social Workers have a caseload of no more than 15 youth and engage in weekly contact with program participants with a minimum of two face-to-face visits per month; one of these being within the participant’s home. When applicable, this position will also supervise the Transitions Advocate, a staff who will provide the day to day support, monitoring, and counseling for each participant in the program.

This position requires dedication, commitment and patience in developing effective case management practices and plans while implementing policies and procedures in a teamwork model and harm reduction approach in accordance with agency policy and California Community Care Licensing Regulations, Title 22, Division 6. This position requires flexibility in personal scheduling and meeting a variety of needs as assigned on a daily basis by the immediate supervisor. Some on call, holiday, night and weekend hours needed on a monthly basis. This employment opportunity requires the employee to live in Sonoma County.

Qualifications:

  • Master’s degree in social work, or a related field preferred with at least one year working with transition aged youth (16-25 years) 
  • BA applicants welcomed with at least 2 years experience working with foster youth and 3 years of case management experience
  • Bilingual in Spanish/English. Bilingual premium available.
  • Must be able to pass a criminal history background check
  • Must be able to drive and transport the youth in employee’s personal vehicle and have a clean driving record and valid CA driver license
  • Meet and maintain the minimum vehicle liability and property insurance limits

Essential Functions:

  • Conduct assessments of client strengths and needs and advocate for them accordingly
  • Aid each youth as they develop and meet the goals established in their goals assessment plan, outlining personal, financial, educational, employment and housing objectives
  • Maintain case records and provide timely reports on an individual participant’s progress and outcomes
  • Perform periodic progress review reports and discharge summaries
  • Provide monthly supervision of participants in their living environment
  • Oversee arrangements for transportation to medical, dental and mental health appointments
  • Monitor participant’s participation in employment, education, and independent living skills training
  • Model and teach positive relationships and boundaries
  • Ensure participant’s compliance with program rules and responsibilities
  • Oversees monitoring of  housekeeping and daily living skills
  • Support problem solving, conflict resolution, and planning counseling
  • Maintain appropriate working relationships with private and public agencies for the purposes of effective case management and collaboration
  • Participate in appropriate meetings as assigned
  • Document all contacts in accordance with agency policy and good case management practice
  • Motivate and maintain effective working relationships with staff and community partners
  • Compliance with all applicable California State Community Care Licensing regulations
  • Provide supervision of the Transitions Advocates
  • Perform individual assessments and carry out appropriate interventions according to case management practice

Skills:

  • Demonstrate capacity to establish rapport, relate to the needs, and motivate youth and young adults.
  • Passion for working with a diverse group of transition age youth including LGBTQI youth, youth with disabilities, and youth from a variety of ethnic and cultural backgrounds
  • Excellent interpersonal skills (verbal and written) with proven ability to facilitate and connect youth and young adults to a broad variety of service providers
  • Understanding of a youth-adult partnership framework
  • Bring an understanding of adolescent development as well as issues specific to transition aged youth and youth in foster care
  • Have an understanding on the effects of complex trauma and relationship to mental health and substance abuse
  • Knowledge of independent living skills including education, employment and housing needs when working with transition age youth
  • A strength-based perspective is essential
  • Relate to other public and private community agency personnel in a positive and supportive manner.
  • Ability to work both collaboratively and independently ~ Generally creative, optimistic, and flexible
  • Stay grounded and make decisions in crisis situations
  • Possess good computer and word processing skills
  • Strong ability to organize and prioritize to balance documentation and youth needs

TLC offers a generous benefit package for full time staff including: 

  • 100% employer paid health, dental and vision plans for the employee. Dependents would be covered at 50% of the base health plan by TLC, dental and vision would be the employee’s responsibility. Coverage is available on the 1st of the month following the date of hire.
  • Paid Time Off: Starting at 14 days per year based on 40 hr. work week. Increases with time of service.

Salary Range: $53,040 – $69,701. Based on education and experience.
